Student Led Conference Materials
Below are the materials students and homeroom teachers will use in order to prepare for each Student Led Conference. Students are encouraged to keep a Google Folder or Google Site to house their reflections using these forms, artifacts, community service and other materials that shows their growth and development within the 21st Century Competencies.

SLC Expectations Form
This is the first form to use during each SLC. The student outlines their information, the date and time of their appointment and who their trusted adult will be. In the beginning of the SLC, the expectations are reviewed and all at the SLC signs the form agreeing to the expectations.

21st Century Competencies Rubric
The NHPS 21st Century Competency Rubric is the main rubric used to support student development in: Problem Solving & Critical Thinking; Accessing & Analyzing Information; Communication & Collaboration; Creativity & Innovation; Initaitve, Leadership & Accountability; and, Citizenship & Responsibility.

Student Success Plan: Part 1, Attendance & Academics
For each SLC, students use this form to note and reflect on their attendance as well as go through each of their classes by noting their grade, reflecting on their evidence of learning and an action plan for each class.

Student Success Plan: Part 2, Social Emotional & Physical Development
Students use this form each SLC to reflect on their Community Service in terms of what they have completed, a plan for completing their hours and monitoring their progress as well as for After School activities and/or community involvement. Additionally, they reflect on their decision making and create new goals to further their development.

Student Success Plan: Part 3, College & Career
In preparation for each SLC, students use this form to reflect and outline what they are currently thinking in terms what their interests are, what college they may want to attend or what they would like to do after graduations as well as what steps they may need to take to meet their goals.
